Each claim from an attorney for compensation and reimbursement of allowable expenses shall be filed with the board on a voucher form approved by the board. Each claim shall be accompanied by a timesheet, in a form approved by the board, detailing:

1. (a) the date of each compensable activity;
2. (b) the purpose of the activity performed;
3. (c) the type of activity performed;
4. (d) the amount of time, in tenths of hours, spent on each activity; and
5. (e) the amount of compensation received for the same services from any other source.
