Before an occupational therapist allows an occupational therapy assistant to work under the occupational therapist's direction, the occupational therapist shall inform the board of the following:

1. (a) The name of each occupational therapy assistant who intends to work under the direction of that occupational therapist;
2. (b) the occupational therapy assistant's place of employment; and
3. (c) the address of the employer.
