1. (a) Before a physical therapist allows a physical therapist assistant to work under the physical therapist's direction, the physical therapist shall inform the board of the following:

   1. (1) The name of each physical therapist assistant who intends to work under the direction of that physical therapist; and
   2. (2) the physical therapist assistant's practice address.
2. (b) Each physical therapist and each physical therapist assistant shall inform the board in writing within 30 days of any changes in the mailing, residence, or practice address.
