Sec. 4. (a) The following fees apply as follows:

(1) For veterinarians, as follows:

(A) Initial application for license is one hundred fifty dollars ($150).

(B) Application for renewal of a biennial license is one hundred dollars ($100).

(C) Application for renewal of license expired for less than three (3) years is one hundred fifty dollars ($150).

(D) Application for renewal of license expired for more than three (3) years is two hundred fifty dollars ($250).

(E) Application for reinstatement of inactive license is one hundred dollars ($100).

(2) For registered veterinary technicians, as follows:

(A) Initial application for registration is thirty dollars ($30).

(B) Application for renewal of a biennial registration is fifteen dollars ($15).

(C) Application for renewal of registration expired for less than three (3) years is thirty dollars ($30).

(D) Application for renewal of registration expired for more than three (3) years is forty-five dollars ($45).

(E) Application for reinstatement of inactive registration is fifteen dollars ($15).

(b) Verification of licensure or registration is ten dollars ($10) and may be subject to regular transaction fees.
