Sec. 10. All approved licensing education or continuing education course providers shall advise the board within thirty (30) days after any significant changes in their operation. Significant changes include, but are not limited to, the following:

1. (1) Going out of business.
2. (2) A change in the address or phone number of the licensing education or continuing education course provider.
3. (3) A change in the name, address, or telephone number of the contact person.
4. (4) Adding a new instructor.
5. (5) Changes in course outline.
6. (6) Any course addition or deletion.
