Sec. 6. (a) Applicants for sponsor approval who do not conduct broker prelicensing courses under IC 25-34.1-5 shall submit the following information:

1. (1) The:

   1. (A) name;
   2. (B) address;
   3. (C) telephone number;
   4. (D) facsimile (FAX) number (if any); and
   5. (E) e-mail address;
2. of the sponsor.

3. (2) The name, address, and resume of the school director responsible for real estate administrative matters, such as the following:

   1. (A) Program development.
   2. (B) Scheduling of classes.
   3. (C) Advertising.
   4. (D) Maintaining facilities and equipment.
   5. (E) Record keeping.
   6. (F) General supervision of the instructional program.
4. (3) If the ownership of the sponsor is a partnership, the names and addresses of the partners.
5. (4) If the ownership of the sponsor is a corporation, the names and addresses of the officers and directors.
6. (5) If the ownership of the sponsor is a limited liability company, the names and addresses of the members and managers.

(b) All applicants for course sponsorship approval shall submit the following:

1. (1) A timed course content outline describing the subjects to be offered during the approval period to include learning objectives for the course.
2. (2) The name and permit number of each course instructor.

3. (3) A statement that the sponsor shall provide the broker who successfully completes an approved course, a certificate of course completion indicating the following:

   1. (A) The name, address, and signature of the sponsor.
   2. (B) The name, address, and license number of the attendee.
   3. (C) The title of the course.
   4. (D) The course location.
   5. (E) The date of the course.
   6. (F) The number of credit hours of continuing education, postlicensing education, and managing broker completed.

4. (4) A statement that:

   1. (A) the sponsor shall conduct an instructor and course evaluation; and
   2. (B) they shall be provided to the commission upon request.
5. (5) The application fee established in 876 IAC 5-3-1.
