Sec. 6. To obtain credit under sections 3.1 through 5.1 of this rule for courses obtained after June 30, 1990, an applicant must have a certificate of course completion, which must:

1. (1) state the applicant's name;
2. (2) contain the name of the sponsoring organization;
3. (3) indicate the course title;
4. (4) identify the course content;
5. (5) state the date and location of the course;
6. (6) certify the course minimum of fifteen (15) classroom hours;
7. (7) indicate hours completed;
8. (8) confirm passing grade; and

9. (9) contain the:

   1. (A) instructor's name;
   2. (B) type of license or faculty position;
   3. (C) license number; and
   4. (D) issuing state.
