Sec. 6. If a psychologist applies for approval of a course the psychologist attended which was not previously approved by the board, the psychologist must submit the following:

1. (1) The name of the sponsor.
2. (2) A description of the course as produced by the course sponsor.
3. (3) The date and location of the course.
4. (4) The names of all presenters and their credentials.
5. (5) Verification of attendance.
6. (6) The number of hours for which credit is requested.
