Sec. 4. The pharmacist in charge or an appropriate designee shall:

1. (1) be responsible for establishing and carrying out a performance improvement program as defined in section 1 of this rule; and
2. (2) develop or be responsible for development of a policies and procedures manual that shall be of sufficient scope and detail that pharmacists practicing in the institutional pharmacy will be able to practice effectively and safely.
