Sec. 2.5. This rule establishes the requirements concerning the applications and fees for issuance or renewal of the following:

1. (1) Licenses for optometrists.
2. (2) Certificates for optometrists to administer, dispense, and prescribe legend drugs as provided for under IC 25-24- 3.
