Sec. 23. The director of the nursing program shall submit an annual report to the board on forms provided by the board. The report shall provide current data on the following:

1. (1) Administrative personnel, credit hours, and faculty to student ratios.
2. (2) Organizational, administrative, and physical changes.
3. (3) Any curriculum changes.
4. (4) Student statistics.

5. (5) A faculty list with a:

   1. (A) completed faculty qualification record for each new member;
   2. (B) supplemental qualification record for each faculty member pursuing a master's degree; and
   3. (C) list of faculty no longer employed by the institution since the last annual report.
6. (6) A clinical agency list and a list of those agencies no longer used since the last annual report.
7. (7) An organizational chart for the nursing program and for the parent institution.
