Sec. 5. The following locations providing health care services are ineligible for inclusion on the health care volunteer registry:

1. (1) A physician's office.
2. (2) An entity licensed or certified by the Indiana state department of health.
3. (3) A health care facility, including a facility that receives federal funding.
4. (4) Any other permanent facility at which the primary purpose is to provide health care services.
5. (5) A residential structure, including long-term care facilities.
6. (6) Any location not meeting the requirements of section 8 of this rule.
