Sec. 10. (a) Each license holder shall inform the agency, in writing, of all changes in address or name within thirty (30) days of the change.

(b) A license holder's failure to receive notification of renewal due to failure to notify the agency of a change of address or name shall not:

1. (1) constitute an error on the part of the agency; or
2. (2) exonerate or otherwise excuse the license holder from renewing the license.
