Sec. 3. Each approved course provider must maintain records of students who successfully complete and pass the course of study for a minimum of two (2) years. The records must include:

1. (1) attendance records;
2. (2) examination score records;
3. (3) student course evaluations; and
4. (4) duplicate copies of completion certificates or the ability to reproduce duplicate completion certificates.
