Sec. 3. As used in this rule, "claim" means a request that benefits of a plan be provided or paid. A claim may be for any of the following:

1. (1) Services (including supplies).
2. (2) Payment for all or a portion of the expenses incurred.
3. (3) A combination of subdivisions (1) and (2).
4. (4) An indemnification.
