Sec. 8. The petitioner must provide the review officer with the following information at least fifteen (15) days before the date of the review:

1. (1) The name and address of any witness who may be called to testify on behalf of the petitioner.
2. (2) All documents or other evidence, whether or not in the possession and control of the petitioner, that the petitioner reasonably expects will be necessary to introduce at the review proceeding.
