Sec. 2. (a) To obtain a license to conduct paid fantasy sports games, a game operator must submit a written application on the form or forms prescribed by the commission.

(b) The application shall include the following information:

1. (1) The name and address of the game operator.
2. (2) The names, addresses, and telephone numbers of the officers of the game operator.
3. (3) The type of allowable paid fantasy sports games the game operator proposes to conduct.
4. (4) Verification that the requirements set forth in IC 4-33-24 have been met.
5. (5) Any other information required by the commission for the purpose of administering this article.

(c) An applicant is under a continuing duty to disclose any changes in the information submitted to the commission.
