Sec. 8. (a) An amateur mixed martial artist is not permitted to participate in an amateur mixed martial arts bout without the following:

1. (1) A mixed martial arts national identification card.

2. (2) Laboratory results, not more than one (1) year old from the date of the event, affirming the negative test results of the amateur mixed martial artist for the following:

   1. (A) Antibodies to the human immunodeficiency virus (HIV).
   2. (B) The surface antigen of the hepatitis B virus.
   3. (C) Antibodies to the hepatitis C virus.

3. (3) From a licensed physician, a written statement, not more than one (1) year old from the date of the event the amateur mixed martial artist seeks to participate in, affirming that the amateur mixed martial artist:

   1. (A) has undergone a thorough medical examination; and
   2. (B) is physically fit and qualified to participate in amateur mixed martial arts.
4. Pre-bout physicals required under section 10 of this rule do not satisfy this requirement.

5. (4) For female amateur mixed martial artists:

   1. (A) laboratory results, not more than five (5) days old from the date of the event, affirming the negative test results for pregnancy; or
   2. (B) two (2) negative over-the-counter pregnancy tests, taken by the amateur mixed martial artist, and verified by the sanctioning body and the event physician, on-site at the event.

(b) It is the responsibility of the sanctioning body of an event to ensure that no amateur mixed martial artist participates in an amateur mixed martial arts bout without the information required in subsection (a).
