Sec. 4. (a) A company that seeks to act as a sanctioning body for an event must first obtain a license as a sanctioning body under IC 4-33-22-18 and this rule.

(b) An applicant for a sanctioning body license must submit the following:

1. (1) A completed sanctioning body application form.

2. (2) Clear photocopies of current government issued photographic identification cards that affirm that:

   1. (A) the principal owner;
   2. (B) substantial owners; and
   3. (C) key persons;
3. are at least twenty-one (21) years of age.

4. (3) One (1) digital photograph of:

   1. (A) the principal owner;
   2. (B) substantial owners; and
   3. (C) key persons;
5. that shows head and shoulders only, without a hat, and in a natural pose.
6. (4) The application fee required under this rule.

7. (5) Information necessary for the executive director or the executive director's designee to conduct a criminal history background check for:

   1. (A) the principal owner;
   2. (B) substantial owners; and
   3. (C) key persons.
8. (6) Additional information requested by the executive director or the executive director's designee that is necessary to determine the applicant's suitability to act as a sanctioning body under IC 4-33-22-18 and this rule.

(c) The following are prohibited from obtaining a license as a sanctioning body:

1. (1) A promoter licensed in any state, country, or tribal nation.

2. (2) An:

   1. (A) employee; or
   2. (B) agent;
3. of a promoter licensed in a state, country, or tribal nation.
