Sec. 8. (a) An activity log must be as follows:

1. (1) Continuously maintained by surveillance employees.
2. (2) Changed with each shift change of employees.
3. (3) Chronological.

4. (4) Contain, at a minimum, the following:

   1. (A) The date and time of each entry.
   2. (B) The identity of the employee making the entry.
   3. (C) A summary of the activity recorded.
   4. (D) Detail of whether the activity was monitored.
   5. (E) Detail of the disposition of copies of recordings of the activity.

5. (5) Include entries for the following events and notifications received by surveillance employees about the events:

   1. (A) The identity of surveillance employees on duty.
   2. (B) Maintenance or repair of a gaming device or money handling equipment.
   3. (C) Live table drop box exchanges.
   4. (D) Electronic gaming device drop bucket exchanges.

   5. (E) Movements or transfers of the following:

      1. (i) Cash.
      2. (ii) Cash equivalents.
      3. (iii) Chips.
      4. (iv) Cards.
      5. (v) Dice.

   6. (F) Detention or questioning of patrons or employees by the security department, including the identity of the following:

      1. (i) Patrons or employees.
      2. (ii) Security department personnel involved.

   7. (G) The beginning, end, and any interruptions of the following:

      1. (i) The soft count.
      2. (ii) The hard count.

   8. (H) Observed:

      1. (i) procedural or control errors; or
      2. (ii) criminal activity.

   9. (I) Pertinent:

      1. (i) telephone calls; or
      2. (ii) radio transmissions.
   10. (J) Malfunctions or repair of surveillance equipment.
   11. (K) Emergency activity.

   12. (L) Surveillance conducted on anyone or any activity that appears:

       1. (i) unusual, irregular, or illegal; or
       2. (ii) to violate IC 4-33, IC 4-35, or this title.

   13. (M) Surveillance conducted at the request of:

       1. (i) a casino licensee;
       2. (ii) personnel of the casino licensee;
       3. (iii) a commission employee; or
       4. (iv) an enforcement agent.
   14. (N) Any other notations deemed necessary by surveillance employees or the commission to ensure compliance with IC 4-33, IC 4-35, and this title.
6. (6) Retained for at least ninety (90) days.

(b) An incident report must be as follows:

1. (1) Made by the person responsible for monitoring the activity.
2. (2) Contain details of an incident observed that involved unusual or criminal activity.
3. (3) Provided to a gaming agent and the executive director or the executive director's designee.
4. (4) Retained a minimum of ninety (90) days.

(c) A visitor's log must do the following:

1. (1) Include the signature of anyone other than surveillance employees on duty who access the surveillance room.
2. (2) Identify visitors.

3. (3) State the following:

   1. (A) The department or agency the visitor represents.
   2. (B) The reason for access to the room.
4. (4) Provide the date and time of arrival and departure from the room.
5. (5) Be retained a minimum of ninety (90) days.

(d) Surveillance room recordings, logs, and reports must be as follows:

1. (1) Retained in a manner to allow them to be easily retrieved by the following:

   1. (A) Time.
   2. (B) Date.
   3. (C) The location of activity.
   4. (D) The type of activity.
2. (2) Provided to an enforcement agent or commission employee immediately upon request.
