Sec. 6. (a) Any major modification to original design initiated by the owner shall, prior to modification, be submitted in writing to the manufacturer or a registered engineer for approval, based on accepted engineering practice. Evidence of approval shall be submitted to the division.

(b) A regulated amusement device that has been subject to a major modification shall have affixed an additional information plate containing the following:

1. (1) The name of the person who performed the major modification.
2. (2) A supplementary serial number.
3. (3) A certification of the date that the work was performed and the maximum capacity of the regulated amusement device.

All regulated amusement devices that were subject to a major modification prior to November 1, 1986, shall have an additional information plate stating that the modification was performed prior to that date.

(c) For purposes of this section, "person" includes the following:

1. (1) An individual.
2. (2) An assignee.
3. (3) A receiver.
4. (4) A commissioner.
5. (5) A fiduciary.
6. (6) A trustee.
7. (7) An executor.
8. (8) An administrator.
9. (9) An institution.
10. (10) A national bank.
11. (11) A bank.
12. (12) A consignee.
13. (13) A firm.
14. (14) A partnership.
15. (15) A joint venture.
16. (16) A pool.
17. (17) A syndicate.
18. (18) A bureau.
19. (19) An association.
20. (20) A cooperative association.
21. (21) A society.
22. (22) A club.
23. (23) A fraternity.
24. (24) A sorority.
25. (25) A lodge.
26. (26) A corporation.
27. (27) A limited liability company.
28. (28) An Indiana political subdivision engaged in private or proprietary activities.
29. (29) An estate.
30. (30) A trust.
31. (31) Any group or combination acting as a unit.
