Sec. 1. (a) An application for approval to deliver and administer a certification course must be submitted to the department not less than fifteen (15) calendar days prior to the commencement of the course.

(b) The application required by subsection (a) must be made in a form prescribed by the department and include the following information:

1. (1) Name of the course.
2. (2) Primary organization requesting the course;
3. (3) Start date and completion date of the course.
4. (4) Maximum number of students in the course.
5. (5) Course syllabus and schedule.
6. (6) Date and time of the written examination.
7. (7) Date and time of the practical skills examination.
8. (8) The proctor's name and contact information.
9. (9) The lead evaluator's name and contact information.
10. (10) The lead instructor's name and contact information.

(c) The department shall approve or deny a complete application required by subsection (a) within ten (10) calendar days of its receipt.
