Sec. 2. (a) Reinstatement of an expired certification may be requested by submitting:

1. (1) a certification reinstatement request containing the information required by the department;
2. (2) documentation that proves the individual completed the required continuing education hours before the expiration of the certification; and
3. (3) a fifty dollar ($50) nonrefundable reinstatement fee.

(b) A certification reinstatement request must be submitted in compliance with a submission procedure established and administered by the department.

(c) The department shall approve certification reinstatement requests that meet the requirements set forth in this rule.
