Sec. 3. (a) Renewal of an expiring certification may be requested by submitting:

1. (1) a certification renewal request containing the information required by the department; and
2. (2) documentation that proves the individual has completed the required continuing education hours before the date the certification expires.

(b) A certification renewal request must be submitted in compliance with a submission procedure established and administered by the department.

(c) The department shall approve certification renewal requests that meet the requirements set forth in this rule.
