Sec. 5.1. (a) Records that have been microfilmed in accordance with this rule may be destroyed or otherwise disposed of only after:

1. (1) the agency files a destruction notice with the oversight committee certifying that the records have been microfilmed in accordance with this rule; and
2. (2) the oversight committee issues a written authorization for the destruction of such records.

(b) The oversight committee shall provide a form for this purpose.
