Sec. 7. (a) A public agency must provide a student with a summary of the student's academic achievement and functional performance, which must include recommendations on how to assist the student in meeting the student's postsecondary goals, when a student:

1. (1) graduates with

   1. a high school diploma as defined in 511 IAC 6-7.1-1(e);
2. (2) graduates with an alternate diploma as defined in 511 IAC 6-7.1-10;
3. (3) exceeds the age of eligibility for special education and related services under this article; or
4. (4) leaves high school with a locally developed certificate, such as a certificate of completion, as identified in 511 IAC 6-7.1-12.

(b) A public agency may provide a student with a summary of performance when the student withdraws from high school after:

1. (1) an exit interview is conducted; and
2. (2) the student's parent and principal consent to the withdrawal as specified in IC 20-33-2-28.5(b).

(c) Leaving high school with a certificate of completion or withdrawal from high school as described in IC 20-33-2-28.5 does not extinguish a student's eligibility for special education and related services.

(d) A summary of performance should include, but not be limited to, the following items:

1. (1) Basic demographic information about the student.

2. (2) Postsecondary goals that:

   1. (A) take into account the student's educational program; and

   2. (B) reflect the:

      1. (i) interests;
      2. (ii) preferences; and
      3. (iii) strengths;
   3. of the student.

3. (3) A summary of the student's academic achievement and functional performance. Information that can be used to prepare the summary includes, but is not limited to, the following:

   1. (A) An academic transcript.
   2. (B) Academic assessment results.

   3. (C) Assessments of functional skills or adaptive behavior that explain a student's ability to:

      1. (i) live;
      2. (ii) work; and
      3. (iii) access the community.
   4. (D) Workforce readiness assessments, career exploration internships, cooperative education experiences, or workforce credentials under IC 20-32-4-4(a)(6)(A) [sic].
4. (4) Recommendations to assist the student in meeting postsecondary goals, including accommodations, modifications, or assistive technology utilized by the student and identified by the student as particularly helpful or necessary to meet academic or functional goals, or both.
