Sec. 4. (a) The public agency must provide to the parent written notice described in subsection (b) not later than five (5) instructional days prior to an initial CCC meeting.

(b) The written notice described in subsection (a) must contain the following:

1. (1) A description and overall findings of each:

   1. (A) evaluation procedure;
   2. (B) assessment;
   3. (C) record; or
   4. (D) report;
2. the public agency used as a basis for any proposed action.
3. (2) A description of action that may be proposed by the public agency.
4. (3) An explanation of why the public agency may propose an action.
