Sec. 5. (a) When a licensed child care center seeks to change its name or corporate status, the following must occur:

1. (1) The center must complete a new application reflecting the revised status.
2. (2) The governing body or its representative must sign and submit the application to the division thirty (30) days prior to the effective date of the changes.
3. (3) The center must attach amended articles of incorporation.

(b) When a licensed child care center has a change of ownership, the following must occur:

1. (1) The new owner shall submit a new application reflecting the revised status and any changes in operation.
2. (2) The owner must provide proof of ownership (bill of sale) within ten (10) days of finalization of the sale.
3. (3) After receiving the application, the division may grant a six (6) month provisional license to the new owner, in order to give the new owner time to obtain regular licensure.
