Sec. 42. Each center shall keep an emergency information file, separate from individual record files, and take it on field trips. It shall contain the following:

1. (1) Emergency numbers for police, fire, hospital, and ambulance.

2. (2) Emergency information for all staff, volunteers, student helpers, and children, including the following:

   1. (A) The name and date of birth.
   2. (B) The name and phone number of spouse, parent, physician, health facility, or other person to contact in an emergency.
   3. (C) A signed authorization for emergency treatment. Parents shall sign this authorization for persons under eighteen (18) years of age. Staff, eighteen (18) years of age or older, shall sign their own authorization.
