Sec. 15. (a) Any youth who is denied admission into the collaborative care program shall be advised by the department in writing of the denial.

(b) Any youth denied reentry into the collaborative care program shall be advised by the department in writing of the denial.

(c) Denials under subsections (a) and (b) shall include:

1. (1) notice of the reason for denial; and
2. (2) notice of the review process for denials, specifying that the youth may make a written request for review, and that the request must include a statement from the youth as to why the youth believes the youth is eligible for the program.
