Sec. 2. The department will issue an annual permit to any carrier who applies and pays an annual permit fee of twenty-five dollars ($25), unless the carrier:

1. (1) holds an unexpired permit;
2. (2) has failed to file a quarterly report required by IC 6-6-4.1-10; or
3. (3) has failed to pay the taxes imposed by IC 6-6-4.1.
