Sec. 4. (a) The division shall issue a conditional certification under this article upon the division's investigation and determination of any of the following conditions:
(b) The time period of a conditional certification shall be determined by the division but may not exceed twelve (12) months. The division shall notify the CMHC of the following:
(c) The division shall reinstate certification if the CMHC meets the requirements in subsection (b)(2).
(d) The division shall terminate the team's certification if the team fails to meet the requirements in subsection (b)(2) within the time period allowed.
