Sec. 21. (a) A comprehensive substance abuse assessment shall be:

1. (1) conducted with each admitted patient; and
2. (2) documented in the patient's record.

(b) The substance abuse assessment shall include, but not be limited to, the following:

1. (1) Completion of a comprehensive substance abuse assessment using a standardized instrument as required in section 6(g)(1) of this rule that indicates the following:

   1. (A) The level of addiction.
   2. (B) The level of care needs.
   3. (C) The intensity of services needed for a patient's recovery.
2. (2) Completion of a biopsychosocial history, including documentation of other treatment that the patient is currently receiving.
