Sec. 8. "Assessment" means procedures by which a counselor and other clinical staff identify and evaluate an individual patient's:

1. (1) strengths;
2. (2) weaknesses;
3. (3) problems; and
4. (4) needs;

in the development and review of the individualized treatment plan that guides treatment.
