Sec. 25.5. "Informed consent" means the following information shall be provided to the patient:

1. (1) A description of any reasonably foreseeable risks or discomforts to the patient.
2. (2) A description of any benefits to the patient that may reasonably be expected from treatment.
3. (3) A disclosure of appropriate alternative procedures or courses of treatment, if any, that might be advantageous to the patient.
