Sec. 1. (a) All centers shall be licensed by the department and shall comply with all applicable federal, state, and local laws and rules.

(b) Components required for licensure as a center are the following:

1. (1) Governing body.
2. (2) Quality assessment and improvement.
3. (3) Infection control program.
4. (4) Laboratory services.
5. (5) Medical records, storage, and administration.
6. (6) Medical staff, anesthesia, and surgical service.
7. (7) Patient care services.
8. (8) Pharmaceutical services.
9. (9) Physical plant, equipment maintenance, and environmental services.
10. (10) Radiology services.

(c) Optional services, not required for licensure, must comply with all rules for that service.

(d) The center shall develop, implement, and maintain a written plan to address the internal review and reporting of unusual occurrences and disasters. This plan must cover, but not be limited to, the following:

1. (1) Patient injuries or marked deterioration of patient condition occurring under unanticipated or unexpected circumstances.
2. (2) Unexplained loss of or theft of a controlled substance.
3. (3) Deaths occurring within the center.
