Sec. 15. Facility operators shall develop a written policy in compliance with this rule that:

1. (1) requires the use of gloves when performing the application of eyelash extensions or any other activity or duty that includes any reasonably anticipated skin, eye, or mucous membrane contact;
2. (2) requires disinfection or sterilization of contaminated reusable items; and
3. (3) provides sanctions, including discipline and dismissal, if warranted, for failure to adhere to the provided policy.
