Sec. 28. Records of each patron shall be maintained by the operator for two (2) years. The record shall include the following, but not be limited to:

1. (1) Patron's name.
2. (2) Address.
3. (3) Age.
4. (4) Date tattooed or body pierced.
5. (5) Design of the tattoo.
6. (6) Location of the tattoo or body piercing on the patron's body.
7. (7) The name of the tattoo artist or body piercer who performed the work.
8. (8) Jewelry or other decoration used.
