Sec. 4. To be eligible for services under this rule, an applicant must meet all of the following:

1. (1) All of the criteria set forth in 405 IAC 2-1.1-6.
2. (2) Be nineteen (19) years of age or older.

3. (3) The applicant has been diagnosed with a BPHC-eligible primary mental health diagnosis including, but not limited to:

   1. (A) schizophrenic disorder;
   2. (B) major depressive disorder;
   3. (C) bipolar disorder;
   4. (D) delusional disorder; or
   5. (E) psychotic disorder.

4. (4) The applicant either:

   1. (A) resides in a community-based setting that is not an institutional setting; or
   2. (B) will be discharged from an institutional setting back to a community-based setting.

5. (5) The applicant meets all of the following needs-based eligibility criteria, as defined in section 2(m) of this rule, based on the following:

   1. (A) Behavioral health clinical evaluation.
   2. (B) Referral form.
   3. (C) Supporting documentation.
   4. (D) The office behavioral health assessment tool results.
