Sec. 7. The member and the member's family have freedom of choice regarding the following aspects of CMHW service delivery:

1. (1) Determining who will participate in the team.
2. (2) Identifying the plan of care goals and the method for achieving those goals.
3. (3) Selecting the CMHW services, as supported by the member's assessment and level of need that will be included in the plan of care.
4. (4) Choosing the office-certified CMHW service provider or providers who will provide, oversee, and monitor implementation of the plan of care.
5. (5) Changing the CMHW service provider or providers at any time during the member's enrollment in the CMHW services program.
