Sec. 2. (a) Chiropractic services are limited to:

1. (1) five (5) visits; and
2. (2) fourteen (14) procedures;

per rolling twelve (12) month period.

(b) Reimbursement is available for medically necessary additional procedures subject to prior authorization.
