Sec. 2. The plan shall be conditioned upon the following:

1. (1) The continued approval of the Healthy Indiana Plan Section 1115 waiver application by the Centers for Medicare and Medicaid Services.
2. (2) The increased federal medical assistance percentage available to individuals defined in 42 CFR 435.119, as provided in 42 U.S.C. 1396d(y).
3. (3) The hospital assessment fee funds as set forth in IC 16-21-10, to support the plan beginning in calendar year 2017.
