Ind. Admin. Code tit. 405, r. 1-1.6-6 – Record keeping and reporting requirements

Sec. 6. The MCO shall maintain a log of all written informal provider objections to determinations and formal provider appeals involving claims. The logged information shall include the:

(1) provider's name;
(2) date of objection;
(3) nature of the objection; and
(4) disposition.

The MCO shall submit quarterly reports to the office regarding the number and type of provider objections and appeals.
