Sec. 4. Certificates of veterinary inspection required under this article shall meet the requirements in 345 IAC 1-1.5-2 and contain the following additional information:

1. (1) An indication whether the animal is or is not a:

   1. (A) scrapie-positive;
   2. (B) suspect;
   3. (C) high-risk; or
   4. (D) exposed;
2. animal.

3. (2) An indication of whether the animal originated in:

   1. (A) an infected;
   2. (B) a source;
   3. (C) an exposed; or
   4. (D) a noncompliant;
4. flock.
