Sec. 2.5. The following qualifies as an owner shipper statement for purposes of IC 15-17 and this title. The statement must be signed by the owner or shipper of the livestock being moved and contain the following information:

1. (1) The location from which the animals are moved.
2. (2) The destination of the animals.
3. (3) The number of animals covered by the statement.
4. (4) The species of animals covered.
5. (5) The name and address of the owner at the time of the movement.
6. (6) The name and address of the shipper.
7. (7) The identification of each animal, unless a rule adopted by the board specifically provides that the identification does not have to be recorded.
