Sec. 2. (a) An application for a certificate of registration as a waste tire transporter must:
(2) contain the information in IC 13-20-14-5(a) and the following information:
(C) A list of all locations where tires will be taken that includes the following:
(b) The fee required by section 3 of this rule must be submitted with the application form when the form is submitted to the department.
(c) An application for a certificate of registration must be submitted as a paper copy unless the commissioner makes a determination that a copy in an electronic format as prescribed by the commissioner and in accordance with IC 13-14-13 is sufficient.
