Sec. 3. The manifest required under section 2 of this rule must include the following information:

1. (1) The amount in tons or pounds of municipal waste transported in the vehicle.
2. (2) The name and address of the solid waste processing facility from which the municipal waste is transported in the vehicle.
3. (3) The destination of the municipal waste.
4. (4) The name and business address of the transporter of the municipal waste.
5. (5) The name and address of the broker involved in the shipment, if applicable.

6. (6) The date of:

   1. (A) the shipment; and
   2. (B) receipt at the final disposal facility.
