Sec. 6. A permit application for a new MSWLF or a lateral expansion must be accompanied by an operational plan in the form of a narrative that describes the proposed MSWLF or lateral expansion. Each narrative must include information on the following:

1. (1) Anticipated quantity, types, and sources of solid waste to be deposited.
2. (2) Equipment to be used for placement and compaction of all solid waste, excavation of soil, movement of stockpiled soils, and application of cover soil.
3. (3) Procedures to control fugitive dust in accordance with the applicable fugitive dust rules in 326 IAC 6-4.
4. (4) Sanitary facilities.
5. (5) A description of the access control to be used.
6. (6) A description of the safety equipment to be used.
7. (7) The distances from the site to the nearest dwelling, factory, and offices.
8. (8) A description of the location, amount, and depth of excavation that will occur at the site.
9. (9) A description of the supervision that will occur at the site.
10. (10) A description of the base flood at the site and whether the site is in the floodway.
11. (11) Proposed hours of operation.
12. (12) The development and progression of the MSWLF or lateral expansion as illustrated in the plot plan required under section 2 of this rule.
13. (13) The quantities of available and necessary cover soil. If cover material is to be obtained from a location other than the site of the proposed MSWLF or lateral expansion, the material's source, quantity, type, and characteristics must be identified.
14. (14) Winter and inclement weather operating procedures, to include methods for obtaining and applying cover soil.

15. (15) A description of protective barriers, leachate, and methane control that includes the following:

    1. (A) Source and type of material utilized.
    2. (B) Method and specifications of construction.
    3. (C) Testing for construction quality control (CQC) and reviews for construction quality assurance (CQA) that will be made part of the reference information contained in the construction contract documents.
16. (16) A description of the sign or signs proposed for the site, as required under 329 IAC 10-20-3.
17. (17) Procedures for the disposal of bulky solid waste such as refrigerators, stoves, fence wire, and other similar items.
18. (18) Procedures for controlling or handling windblown materials.
19. (19) Procedures to be used to prevent and extinguish fires.
20. (20) Details of salvage operations, if planned, indicating how the salvage operation will comply with 329 IAC 10-20-6.
21. (21) Facilities for personnel and equipment.
22. (22) Storage, treatment, and disposal methods for leachate.
23. (23) Run-on and run-off control system as specified in 329 IAC 10-20-11.
24. (24) Erosion and sedimentation control measures as specified in 329 IAC 10-20-12.
