Sec. 3. A permit including a renewal permit must be issued for a fixed term not to exceed five (5) years in accordance with IC 13-15-3-2. A permit may be modified or revoked prior to the expiration of the term for cause as provided in section 6 of this rule or in accordance with conditions set forth in the permit and IC 13-15-7-1.
