Sec. 4. (a) Any owner, operator, or permittee required to monitor under this article or by any permit issued under this article shall maintain all records of all monitoring information and monitoring activities, including:

1. (1) the date, exact place, and time of the sampling measurements;
2. (2) the sampling methods used;
3. (3) the person or persons who performed the sampling or measurements;
4. (4) the date or dates analyses were performed;
5. (5) the person or persons who performed the analyses;
6. (6) the analytical techniques or methods used;
7. (7) the results of such measurements or analyses; and
8. (8) all quality assurance/quality control documentation.

(b) The owner, operator, or permittee of a solid waste land disposal facility shall record and retain at the facility in an operating record, or, in an alternative location approved by the commissioner, any records required by this article.

(c) All records of monitoring activities required by this article and results thereof shall be retained by the owner, operator, or permittee of a solid waste land disposal facility for three (3) years unless otherwise specified in this article. The three (3) year period shall be extended:

1. (1) automatically during the course of any unresolved litigation between the commissioner and a permittee of a solid waste land disposal facility; or
2. (2) as required by the permit conditions.

(d) Information submitted to the department to meet a requirement of this article must meet the following standards:

1. (1) All drawings, plans, maps, and documentation must be properly titled and must include the following where applicable:

   1. (A) The date and author of each drawing, plan, or map.

   2. (B) Documentation of the coordinate system of the drawing, plan, or map, including the following:

      1. (i) Measurement units.
      2. (ii) Datum.
      3. (iii) Identification of the coordinate system that was used, such as the Universal Transverse Mercator or the State Plane coordinate system.
   3. (C) A bar scale on each drawing, plan, or map.
   4. (D) Elevations that correlate with United States Geological Survey mean sea level data.
   5. (E) The facility name.
   6. (F) The state regulatory identification number, such as a permit number or authorization number.
   7. (G) The facility's United States Environmental Protection Agency identification number, if available.
   8. (H) A north arrow.
   9. (I) A map legend.

2. (2) Submittals of sampling and monitoring results must include the following:

   1. (A) Results of laboratory analyses.
   2. (B) Results of field measurements, including water elevations and well depths if applicable.
   3. (C) Laboratory name.
   4. (D) Date of the sampling or monitoring event.
